Ordering Service In most departments, there are staff members (usually facilities coordinators) who serve as liaisons for ordering telephone and network services. While we make every effort to provide detailed information here on the Web about our services, when it comes time to order a service or make a change to an existing service, your facilities coordinator is your starting point. For more information on ordering services, please contact us: Phone: (831) 459-4255 (ext. 9-4255) Standard Orders- Ethernet Jack Activation, Disconnect, Move or Upgrade You can now order telephone and network service for a workstation with one form. To order, you must be a pre-approved contact with signature authority, and supply billing codes for service. Please allow 10 business days for your work to be completed. Mac Users: These forms do not work correctly with the Mac OS X Preview utility.
To fill out the forms on a Mac running OS X, please use Adobe Acrobat Reader. Please Note: The recharge form has been replaced with these forms except in the case of new wiring or small cable projects.
PLEASE NOTE: We currently require an actual signature on these forms. While the forms can be filled out on line, they must be printed and either faxed or mailed. We are working on a form that can be submitted electronically. Service requests are scheduled on a first come, first served basis. To secure the earliest available date, please fill-in the date field of the order form with ASAP. If you require service on a specific date, and want that date guaranteed, we must receive your order a minimum of ten business days prior to the date you write in the date needed field. Expedited orders can be negotiated, but may result in additional charges. NOTE: For temporary telephone or network service, such as for a short-term conference, please use the Installation form. Make a note in the comments section stating the start and end dates of the temporary service. All regular installation and disconnect charges will apply, but you will be billed a prorated monthly service charge for the days used only. New Wire or Small Cable Projects Where the building cable is not sufficient to support services, new wiring can be installed as a special construction project. Projects include new data or telephone wiring and other unique/special network move, add, change work.
