Account Forms

The following information has been provided to assist you make a request for a new account.
These forms are used primarily by staff and faculty, however there may be some instances when a student
may be required to fill out and submit one of the forms below.

New UCSC Email Account for Staff and Faculty

A form is required for all new email account requests for staff, faculty and temporary staffing appointments. The requestor will need an account claim code for the account. Before we can process your request we must have the requestor's signature. Please contact the Help Desk for instructions for obtaining this form.

Email Accounts for Students

Students are assigned a CruzID upon enrollment. Students can activate their pre-assigned CruzID through the web portal at MyUCSC.

How to Make Changes to Your Account

Account Modification Form (PDF)
The account modification form is used by students, staff, faculty and temporary staffing appointments to modify aspects of a user account or to add administrative account access after the initial UCSC Account has been established.

Students requesting an increase in modem time or disk space will need the signature of a faculty member before the request can be processed. Campus staff/faculty/temporary appointments who are requesting access to an administrative system will need the signature of their supervisor (Department Chair for faculty). If you have been issued a UCSC Account in the past (via employment or as a student) your UCSC login name will be the one that you had previously.

To ensure speedy processing of all account applications, make sure all forms are filled out completely and correctly.

PPS Accounts
These forms are required for PPS accounts for staff, faculty and temporary staffing appointments:

These forms are required for NES accounts for staff, faculty and temporary staffing appointments:

AIS Accounts
These forms are required for AIS accounts for staff, faculty and temporary staffing appointments.

Complete all required fields at the top of the AIS Supplemental Form and sign the form at the bottom. Complete only those AIS functional areas for which access is being requested. This form includes a section for Advising and replaces the Advising Supplemental Form

FIS Accounts
These three forms are required for a FIS account for staff, faculty and temporary staffing appointments:

Financial Information Systems Access Request Form (PDF) The above form must be submitted to the ITS Support Center either by mail to 54 Kerr Hall, for by fax to 94171 for processing. You will be notified via email when your password is ready to pick up.

CruzBuy Accounts
CruzBuy Profile Authorization Form (PDF)
Complete the CruzBuy Profile Authorization form, obtain required signatures and mail to CruzBuy Account Administration at mail stop: Accounting Office, or fax to 459-3601. Any questions can be directed to fis_probs@ucsc.edu. You will receive an email when your account is ready with instructions for password pick up.

Supplemental Approvals (PDF)
Use the form at this link to add special approvals to your existing account such as Food & Entertainment, Student PO Approve/Print, and as directed by CruzBuy Account Administration.

Sundry (Limited) Accounts
Sundry Account Form (PDF)
Sundry Accounts are limited access accounts which allow access to the campus wired and wireless network, campus email, and unix timeshares. Individuals who qualify for a Sundry Account are; Emeriti Faculty, Concurrently Enrolled students, Extension students, Summer Session students, and individuals/agencies officially affiliated with UCSC (contracting agencies). Such qualification of proven affiliation is at the discretion of ITS. Eligibility is subject to compliance with University policy.

For Infoview report-reading or report-writing access to Data Warehouse, please complete the forms listed above. There is no fee for a report-reading account. Report-writing accounts require either a Transfer of Expense form to purchase a new license for $650, or a note on the account form stating the pre-existing license that is being transfered. Signature requirements are stated on the Data Warehouse Account Form and also at the Data Warehouse Subject Specific Access. Training is not required before account access is granted.
